Sweetwater County File of Life® Project – webinar

Join us for the opportunity to learn more about the File of Life^® program. The File of Life^® is a small personal health record that can be kept on your refrigerator. Once the history card is filled out, it is placed in a magnetic sleeve on the outside of the refrigerator in a visible place.

To participate:

1. Click or Copy and Paste this URL to your web browser: https://qualitynet.webex.com. This should bring you to the QualityNet e-University webpage.
2. Locate the event you wish to join and click “Join” (located to the right of the event title) .
3. Enter your name and email address as prompted .
4. Enter the password: fileoflife
5. The automatic system set-up should start at this point. If a dialogue box appears, click run. Please note, the automatic system set-up does take a few minutes to complete.
6. Dial in to the teleconference. The number is 1-866-612-6838. The access code is 837860.
