Improving transitions of care and care coordination benefits the patient\u2019s overall health, improves medication safety and decreases the incidence of adverse drug events.<\/p>\n<p>Mountain Pacific works with health care communities to improve care transitions and reduce the number of unnecessary hospital admissions and readmissions.
