Utilizing the Comprehensive Primary Care Payment (Track 2 Practices)

The last blog discussed Track 2 practices changing the Comprehensive Primary Care Payment (CPCP) ration in 2018. This hybrid payment is designed to be used to provide alternate visit types for patients across the practice. Why is it important to provide these services to patients and where does a practice begin? Let’s explore different options for success in offering alternate visits.

Where to begin?

  • Use data to understand the needs of the patients
    • Surveys
    • Patient Family Advisory Council feedback
    • Suggestion box
  • Assess the practice environment
    • Staff skill levels
    • Ability to dedicate time to a new project
    • Location for televisits, group visits, etc.
  • Identify opportunities for success based on patient needs
    • No public transportation could increase telemedicine visits
    • A large population in a skilled nursing facility could benefit from nursing home visits
    • A large diabetic population could benefit from group visits for peer support

What are the benefits?

  • Increased patient access
  • Better patient and staff engagement
  • Replace office visits for certain visits
  • Save time per visit

Remember, not every alternate visit type is going to fit perfectly into your practice. It is important to take in to account care team and patient feedback. Don’t forget to start small, plan out change and analyze effectiveness. CPC+ Connect offers an Action Group entitled “Going Beyond the Traditional Office Visit with Your CPCP”.
