The end of first quarter but Q1 reporting is quickly approaching. Before we get in to the meat of the post I want to put minds at ease and assure you that you <strong>do not<\/strong> need to report eCQMs quarterly. These are reported annually. So, what do practices need to report?<\/p>\n<p><strong>Care delivery reports<\/strong> are done quarterly and focus on the CPC+ \u201cdrivers\u201d; access and continuity, targeted care management, comprehensiveness and coordination, patient and caregiver engagements, and planned care and population health.<\/p>\n<p>The intent of these care delivery reports is to monitor a practice\u2019s progress over time. For Care Delivery Reporting, you will answer a list of questions for each driver. You don\u2019t need to have positive answers or answers for every question. Answer them based on the progress the practice is making in the program.<\/p>\n<p><strong>Requirements for the 1st Quarter Reporting:<\/strong><\/p>\n<ul>\n<li>Care Delivery Reporting data to be entered into the CMS Practice Portal for each organization<\/li>\n<li>Q1 reporting period 1\/1\/2017-3\/31\/2017<\/li>\n<li>Q 1 reporting due by April 14th(If extensions are needed please contact me)<\/li>\n<li>Reporting on the following \u201cdriver\u201d elements are required for Q1: (see the CMS 2017 CPC+ Care Delivery Reporting Guide for the list of questions you will address for Q1 reporting for each item below)\n<ul>\n<li>Access and Continuity\n<ul>\n<li>24\/7 Access<\/li>\n<li>Empanelment<\/li>\n<li>Continuity of Care<\/li>\n<\/ul>\n<\/li>\n<li>Targeted Care Management\n<ul>\n<li>ID patients for management<\/li>\n<li>Care management staffing<\/li>\n<li>Coordinating discharge and ED<\/li>\n<li>Medication management<\/li>\n<\/ul>\n<\/li>\n<li>Comprehensiveness and Coordination\n<ul>\n<li>ID and communicating with hospitals and EDs<\/li>\n<li>Care compacts\/agreements<\/li>\n<\/ul>\n<\/li>\n<li>Patient and Caregiver Engagement\n<ul>\n<li>Patient and Family Advisory Council<\/li>\n<li>Self Management support for selected conditions<\/li>\n<\/ul>\n<\/li>\n<li>Planned Care and Population Health\n<ul>\n<li>Team-Based Care<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><strong>Data to prepare for Care Delivery Reporting for the first quarter:<\/strong><\/p>\n<p><strong>As part of the reporting, you will also be required to provide the following information.
