Change can be difficult, especially when processes have been the same for an extended period.  But Mountain-Pacific can help. Below are some useful strategies to help you work through changes in your practice by mapping workflows and monitoring the effectiveness of your efforts.  The primary care functions in CPC+ may not be new to your practice\u2014it is critical however, to utilize your care team\u2019s  time to its fullest potential.  By utilizing a workflow mapping process, you can examine how things are done and ensure you are maximizing your care team.<br \/>\n<strong>When to use workflow mapping for an existing process: <\/strong><\/p>\n<ul>\n<li>If the current process is wasteful (time, supplies, etc.)<\/li>\n<li>If data reports are incorrect<\/li>\n<li>To populate data report (i.e., the correct data entry to populate an eCQM or custom report) <\/li>\n<li>If there is significant EHR documentation change (risk stratification, care plans, patient education, etc.)<\/li>\n<li>If the patient\/staff are dissatisfied with current process\u2014 \u201cToo many clicks!\u201d<\/li>\n<\/ul>\n<p><strong>Examples: <\/strong><\/p>\n<ul>\n<li>Care team members don\u2019t have enough time to screen patients for depression <\/li>\n<li>Data reports aren\u2019t reflecting correct data<\/li>\n<li>Patient check-in takes too long<\/li>\n<\/ul>\n<p><strong>When to use workflow mapping for a new process: <\/strong><\/p>\n<ul>\n<li>If there is significant change in flow of care delivery <\/li>\n<li>If there is significant change in documentation of care delivery <\/li>\n<li>If there is an addition of new care team providers<\/li>\n<li>If there are new regulatory requirements <\/li>\n<\/ul>\n<p><strong>Examples: <\/strong><\/p>\n<ul>\n<li>New EHR technology to document care plans<\/li>\n<li>Patient risk stratification<\/li>\n<li>Closing the referral loop workflow development<\/li>\n<\/ul>\n<p><strong>Layers of workflow: <\/strong><\/p>\n<ol>\n<li>Physical <\/li>\n<ul>\n<li>Includes environmental layout of patient room, equipment, devices, supplies, etc. <\/li>\n<\/ul>\n<li>Electronic <\/li>\n<ul>\n<li>How is the work documented? What screens and fields are used? <\/li>\n<\/ul>\n<li>Data <\/li>\n<ul>\n<li>Where does the documented information go? <\/li>\n<li>Why does it go there (triggers or reports)? <\/li>\n<li>How does it get there (interfaces, uploads, etc.) <\/li>\n<\/ul>\n<\/ol>\n<p>Steps to workflow mapping: <\/p>\n<ul>\n<li>Map the \u201cAs is\u201d process\u2014What are you doing right now?<\/li>\n<li>Analyze the \u201cAs is\u201d process\u2014Are you meeting CPC+ requirements in the most efficient way for your practice?<\/li>\n<li>Create the \u201cTo Be\u201d process\u00ac\u2014What do the people doing the work think could be improved upon? <\/li>\n<li>Identify points of change and what the change will look like\u2014Communicate all changes to the team!<\/li>\n<\/ul>\n<p>Map the part of the process that is: <\/p>\n<ul>\n<li>Measurable <\/li>\n<li>Most directly affects the overall outcome <\/li>\n<\/ul>\n<p>Learn more about workflow analysis and eCQI resources <a href=\"http:\/\/www.mpqhf.org\/corporate\/health-and-technology-services\/resources\/ecqi-resources\/\" target=\"_blank\">here<\/a>.<\/p>\n<p>Also, please refer to CPC+ Connect to network with other CPC+ practices utilizing the same EHR for ideas and best practices.  We are in the process of setting up regional pages for both Montana and Hawaii in the CPC+ Connect Portal and will let you know when that is complete.
