We first opened our doors in Helena, Montana, in 1973; since then, we have broadened our reach to include Wyoming, Alaska, Hawaii and the U.S. Pacific Territories of Guam, American Samoa and the Commonwealth of the Northern Mariana Islands.<\/p>\n<p><strong>Quality Innovation Network-Quality Improvement Organization (QIN-QIO)<\/strong><\/p>\n<p>Part of what we do is under the direction of the Centers for Medicare &amp; Medicaid Services (CMS). We partner with health care providers, practitioners, stakeholders, patients and families on a variety of quality improvement initiatives to achieve better care, better population health and lower health care costs. These initiatives include:<\/p>\n<ul>\n<li><a href=\"https:\/\/dsiohn098w.mpqhf.org\/QIO\/hospital-infections\/\" target=\"_blank\" rel=\"noopener\">Preventing healthcare-associated infections<\/a> in hospitals and nursing homes<\/li>\n<li><a href=\"https:\/\/dsiohn098w.mpqhf.org\/QIO\/conditions-in-nursing-homes\/\" target=\"_blank\" rel=\"noopener\">Preventing healthcare-acquired conditions<\/a> related to long-term care in nursing homes<\/li>\n<li>Improving the <a href=\"https:\/\/dsiohn098w.mpqhf.org\/QIO\/improving-care-coordination\/\" target=\"_blank\" rel=\"noopener\">coordination of care<\/a> from one health care setting to another and reducing unnecessary hospital admissions and readmissions<\/li>\n<li>Improving <a href=\"https:\/\/dsiohn098w.mpqhf.org\/QIO\/cardiac-health\/\" target=\"_blank\" rel=\"noopener\">cardiovascular health<\/a><\/li>\n<li>Improving the health and wellbeing of people with <a href=\"https:\/\/www.mpqhf.org\/QIO\/diabetes\/\" target=\"_blank\" rel=\"noopener\">diabetes<\/a><\/li>\n<\/ul>\n<p><strong>Montana Medicaid<\/strong><\/p>\n<p>Mountain Pacific provides <a href=\"https:\/\/www.mpqhf.org\/corporate\/medicaid-utilization-review\/\" target=\"_blank\" rel=\"noopener\">Medicaid utilization review<\/a> and management services for the Montana Department of Public Health and Human Services. We have been performing medical case review for specific Medicaid contracts since 1974.
