Wyoming Long-term Care Facilities Receive Quality Health Care Awards
Casper, Wyo., (Aug. 10, 2017) — Mountain-Pacific Quality Health is proud to announce the winners of the 2017 Nursing Home Quality Awards. Ten Wyoming nursing homes received the top honor—the Recognition of Quality Excellence Award. Ten facilities received the Recognition of Quality Achievement Award and two facilities received the Innovator Quality Achievement Award.
All honored facilities met the quality goals set for these awards and received their awards on August 10th in Casper during the Wyoming Quality Health Care Conference.
The Centers for Medicare & Medicaid Services (CMS) supports the National Nursing Home Quality Care Collaborative (NNHQCC) through the Quality Innovation Network-Quality Improvement Organization (QIN-QIO) program. Mountain-Pacific is the local facilitator for Wyoming.
The NNHQCC’s goals are to instill quality and performance improvement practices, eliminate healthcare-acquired conditions and improve resident satisfaction. To measure how well nursing homes are achieving these goals, CMS created a “composite score” system, which is based on 13 quality measures—the lower the score, the better. Mountain-Pacific’s Quality Awards are based on each facility’s composite score, its licensure with Wyoming’s state survey agency, the Office of Healthcare Licensing and Survey, and its certification with CMS.
“These awards truly demonstrate the commitment and dedication of these facilities to deliver high-quality care to their residents and families,” said Pat Fritz, Mountain-Pacific’s state director for Wyoming. “We are honored to partner with Wyoming’s long-term care facilities to help promote the ideology that every resident receives the safest, highest quality of care possible.”

• Ivinson Memorial Hospital Extended Care Unit, Laramie
• Westview Health Care Center, Sheridan
• Douglas Care Center, LLC, Douglas
• Cheyenne Regional Medical Center TCU, Cheyenne
• Life Care Center of Casper, Casper
• Morning Star Care Center, Fort Washakie
• Star Valley Care Center, Afton
• South Lincoln Nursing Center, Kemmerer
Ten facilities received the Recognition of Quality Achievement Award:
• Bonnie Bluejacket Memorial Nursing Home, Basin
• The Legacy Living and Rehabilitation Center, Gillette
• Sheridan Manor, Sheridan
• New Horizons Care Center, Lovell
• Rawlins Rehabilitation and Wellness, Rawlins
• Rocky Mountain Care – Evanston, Evanston
• St. John’s Nursing Home, Jackson
• Thermopolis Rehabilitation and Wellness, Thermopolis
• Platte County Legacy Home, Wheatland
• Green House Living for Sheridan, Sheridan
Two facilities received the Innovator Quality Achievement Award:
• Weston County Manor Health Services, LTC, Newcastle
• New Horizons Care Center, Lovell
All Wyoming long-term care facilities are eligible for the Nursing Home Quality Awards. Mountain-Pacific thanks all Wyoming long-term care facilities for their dedication to providing safe, quality care.
