MIPS and APM: Documentation Preparation – Like Pudding

An audit is the examination of documents and processes to verify compliance to a set of rules and/or standards during a specific period of time. For MACRA, the time period will be equivalent to your reporting period.

The proof is in the pudding and your audit file should be just that – a bowl full of pudding that supports how each submitted quality or performance measure was met in accordance with the program rules. Although there is no instant pudding mix to creating your audit file, timeliness and completeness are the secret ingredients for this recipe.

Remember, pudding needs to be made in advance because it requires refrigeration before eating. Similarly, your audit file should be ready months in advance of when you may be audited. This will help in two ways:

1. You won’t have to remember specific details about what you did months or years ago because it will already be documented, and
2. The deadline to submit documents to the auditor will be manageable, especially if the request for audit was sitting in someone’s inbox while they were on vacation for a few weeks. This is why it is so important to provide CMS with dual contact information.
