Practice care delivery report data incorrect? We’re here to help, you can request corrections.

Quarter 1 reporting is closed. Congratulations to all who have completed the first reporting requirement! So, what do you do if you missed an answer, fat-fingered a response, or forgot something? Have no fear! Nothing is set in stone and you can request a correction. Here is a timeline for correction requests for 2017 reporting.
Steps to request an extension or a correction
- Log-in to the CPC+ Practice Portal
- Click on the “Practice Reporting” tab and scroll to the bottom of the page
- Click on the request button and complete the form
- Click on the “Save” button to submit request
**I advise taking a screen shot with the date the request was made and keep for your records or email Kristen Schuster for assistance.
