• • This learning module provides an overview and knowledge on the topic of health equity. You can watch the segments in order to review the foundational basics of health equity, or access individual segments that focus on your immediate needs. This learning module was produced by Mountain Pacific in collaboration with our Regional Chronic Disease Collaborative partners in Alaska, Hawaii, Montana and Wyoming.

• • This learning module provides an overview of health care quality improvement basics based on the Model for Improvement, developed by Associates in Process Improvement and recommended by the Institute for Healthcare Improvement (IHI). The topics covered in this series include how to establish specific, measurable, attainable, relevant, time-bound, inclusive and equitable (SMARTIE) goals, the use of the Plan Do Study Act (PDSA) cycle for testing improvements, addressing inclusion and health equity and how to monitor and track data to support your health care quality improvement efforts.
